Deliver high-quality patient care in a dynamic per-diem role focused on home health risk assessments. As a Nurse Practitioner specializing in home health, you'll serve Medicare Advantage plan members across Barry County, with visits in communities such as Monett, Savannah, Country Club, and Fillmore. Key Responsibilities

• Perform comprehensive annual or post-discharge risk assessments during 45-minute home visits
• Review and evaluate patients prior and current medical history, ongoing conditions, and medication profiles
• Educate patients on chronic conditions and proper medication usage
• Identify gaps in care and make recommended interventions to optimize patient outcomes
• Documentation and assessment onlyno ongoing treatment or prescription responsibilities

Qualifications

• Active Nurse Practitioner license in Missouri
• Current BLS/CPR certification
• Board certified via AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months recent Nurse Practitioner experience (preferred)

Your commitment and expertise will be rewarded at a rate of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
