Take on a pivotal healthcare role conducting in-home risk assessments in Vanderburgh County, Indiana. This per diem opportunity requires a skilled Nurse Practitioner experienced in home health and preventive care.
Schedule:
- Flexible shifts available any day, Monday through Sunday

As a Nurse Practitioner in this assignment, you will visit Medicare Advantage plan members in areas such as Evansville and Darmstadt, with each home health risk assessment averaging 45 minutes. Assignments may range from annual health reviews to post-discharge evaluations, tailored to the specific needs of each patient.
Core Responsibilities:
- Complete annual in-home health risk assessments for Medicare Advantage members
- Assess medical history, current conditions, and review medication usage
- Educate patients regarding their health conditions and prescribed medications
- Identify gaps in care and recommend appropriate resources or actions
- No creation of treatment plans or medication prescribing required
Essential Qualifications:
- Active and unrestricted Nurse Practitioner license in Indiana
- Masters degree in Nursing (MSN) from an accredited institution
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months experience as an NP preferred
- Background in home health beneficial
This per diem role offers compensation of $80-$90 per evaluation, plus a mileage stipend and payment for administrative time.
