This per diem opportunity is ideal for experienced Nurse Practitioners passionate about home-based care. In this role, you will perform comprehensive home health risk assessments for Medicare Advantage plan members throughout Marion County, including the areas of Centralia, Salem, and Sandoval. Each evaluation typically lasts around 45 minutes and may include annual health assessments or post-discharge visits, contributing to improved patient outcomes in your community.

- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage enrollees
- Review and evaluate patients prior and current medical history, medical conditions, and medication records
- Educate patients on their health conditions and proper medication management
- Identify gaps in care and present recommendations to address them
- No creation of new treatment plans or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR certification (BLS)
- Board Certification by AANP or ANCC
- Six months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
