Seize the opportunity to make a direct impact in community health as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers you true flexibility, with the ability to design your own schedule and cover shifts between Monday and Sunday. Assignments are available throughout Fayette County, including Washington Court House, Jeffersonville, Bloomingburg, and Milledgeville, serving Medicare Advantage plan members in their homes.
Schedule/Hours
- Flexible shifts available Monday through Sunday
- Create your own schedule; self-directed shifts
- Set minimum weekly or bi-weekly hourly commitments
Key Responsibilities
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Assess medical history, current health conditions, and medications
- Educate patients about their conditions and prescribed medications
- Identify gaps in care and recommend solutions
- No requirement to create treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner license in Ohio
- Board certified through AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- Minimum of 6 months current experience as a Nurse Practitioner preferred
This opportunity provides $80$90 per completed evaluation, as well as compensation for mileage and administrative work related to your visits.
