Take on a vital role as a Nurse Practitioner specializing in home health, conducting risk assessments for Medicare Advantage members in Fayette County, Ohio, including Washington Court House, Jeffersonville, Bloomingburg, and Milledgeville. This per diem opportunity allows you to make a direct impact by providing thorough, patient-centered assessments in the comfort of each member's home.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments to support a Medicare Advantage plan
- Assess patients prior and current medical history, ongoing conditions, and medication usage
- Deliver clear education regarding conditions and medication to patients
- Identify gaps in care and make recommendations to enhance patient outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC required
- At least six months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Enjoy compensation of $80-$90 per evaluation, with additional mileage stipend and administrative time factored in.
