Job Description

Delivering high-quality care in a flexible, rewarding environment, this per diem opportunity offers Nurse Practitioners the ability to conduct home health risk assessments within Surry County. With a focus on Medicare Advantage plan members, you will have the chance to use your clinical expertise across Mount Airy, Elkin, Dobson, and surrounding communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule to suit your lifestyle
  • Commit to a minimum number of hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform comprehensive home health risk assessments (approximately 45 minutes per visit)
  • Evaluate patients prior and current medical and medication histories
  • Educate patients on chronic conditions, treatment options, and medication adherence
  • Identify gaps in care and make recommendations for follow-up or further medical attention
  • Focus on patient assessment only no prescriptive authority or treatment planning required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred (but not required)
