Job Description
Deliver high-quality, patient-centered care as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Surry County, North Carolina. This flexible, per diem opportunity is well-suited for Nurse Practitioners looking to design their own schedule while making a meaningful impact in community-based care.
Schedule:

Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate past and current medical histories, health conditions, and medication regimens
- Educate patients regarding their medical conditions and medication management
- Identify gaps in care and recommend appropriate next steps to optimize patient health
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina required
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of NP experience preferred
- Proven ability to deliver compassionate and thorough care in a home setting
This role offers compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time. Youll have the flexibility to manage your own caseload and make a tangible difference for patients in Mount Airy, Elkin, Dobson, and surrounding communities.
