Delivering high-quality care in a flexible, rewarding environment, this per diem opportunity offers Nurse Practitioners the ability to conduct home health risk assessments within Surry County. With a focus on Medicare Advantage plan members, you will have the chance to use your clinical expertise across Mount Airy, Elkin, Dobson, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule to suit your lifestyle
- Commit to a minimum number of hours on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive home health risk assessments (approximately 45 minutes per visit)
- Evaluate patients prior and current medical and medication histories
- Educate patients on chronic conditions, treatment options, and medication adherence
- Identify gaps in care and make recommendations for follow-up or further medical attention
- Focus on patient assessment only no prescriptive authority or treatment planning required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred (but not required)
