Job Description

Deliver personalized care and make a meaningful impact by conducting home health risk assessments for Medicare Advantage plan members residing within Latah County. As an experienced Nurse Practitioner with a focus on home health, you will visit clients in their residences across the regionincluding Moscow, Genesee, and Troy. Each assessment averages 45 minutes and may involve annual health reviews or post-discharge evaluations, allowing you to utilize your clinical expertise while supporting ongoing patient wellness in a flexible, per diem capacity.

  • The ability to create and manage your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members in their homes
  • Review and document prior/current medical history, conditions, and all medications
  • Identify care gaps and provide actionable recommendations (no treatment planning or prescribing required)
  • Deliver patient education regarding medical conditions and medications
  • Ensure thorough and compassionate evaluations within the allotted time

Qualifications:

  • Valid Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • National Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred
