Job Description
Deliver high-quality care as a Nurse Practitioner specializing in home health risk assessments in Peoria County, Illinois. This per diem position allows you to customize your schedule, taking on flexible shifts between Monday and Sunday with minimum weekly or bi-weekly commitments. Work independently, conducting 45-minute visits for Medicare Advantage plan members in various locations within Peoria County, including Peoria, Chillicothe, and Bartonville.
Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage members.
- Review and evaluate medical history, chronic conditions, and medications.
- Educate patients on disease management, medications, and preventive measures.
- Identify and address gaps in care, providing recommendations for resolution.
- No creation of treatment plans or medication prescribing involved.
Schedule:
- Flexible shifts available Monday through Sunday.
- Build your own schedule with the option to select consistent weekly or bi-weekly hours.
- Assignments may occur anywhere within Peoria County on a given shift.
Qualifications:
- Licensed Nurse Practitioner in the state of Illinois.
- Active Board Certification through AANP or ANCC.
- Current CPR (BLS) certification.
- MSN from an accredited nursing program.
- At least 6 months of current nurse practitioner experience preferred.
