Job Description

This per diem opportunity involves conducting comprehensive risk assessments for Medicare Advantage plan members across Peoria County, Illinois. Youll visit patients in their homes, enabling them to receive high-quality care and guidance in a familiar setting. Flexible scheduling lets you design your own workweeks with coverage needed across areas such as Peoria, Chillicothe, and Bartonville.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to create your own week
  • Commitment can be weekly or bi-weekly, with a minimum hour requirement

Responsibilities:

  • Conduct thorough annual home health risk assessments in member residences
  • Review and document medical history, conditions, and medication usage
  • Offer patient education about diagnoses and prescribed medicines
  • Recognize and address any existing gaps in patient care, making recommendations as necessary
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner license in Illinois
  • CPR certification (BLS) required
  • Board Certified via AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • MSN from an accredited nursing program

Compensation and Benefits: You will earn between $80 and $90 for each completed evaluation. A mileage stipend and compensation for administrative time are also provided, rewarding your commitment to delivering essential care services.
