Job Description
As a Nurse Practitioner specializing in home health, you'll take on an essential role conducting risk assessments for Medicare Advantage plan members in Durham County, North Carolina, including areas such as Raleigh, Morrisville, and Cary. This per diem position offers the flexibility to choose your schedule and locations for each shift, providing a unique opportunity to make a real impact in patients lives from the comfort of their own homes.
Schedule/Hours:
- Flexible shifts available between Monday and Sunday
- Create your own schedule to fit personal or professional commitments
- Minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Conduct comprehensive annual home health risk assessments for plan members
- Evaluate patients prior and current medical history, conditions, and medications
- Deliver thorough education to patients regarding their health concerns and medication regimens
- Detect and address potential gaps in care, offering recommendations for follow-up
- Focus exclusively on assessmentsno creation of treatment plans or medication prescribing
Qualifications and Preferred Experience:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Valid CPR (BLS) certification
- MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
