This per diem position offers the flexibility to create your own schedule, allowing you to balance your professional and personal commitments with ease. Youll work in communities such as Newport and Parrottsville, delivering essential evaluations that help improve patient outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own custom schedule
- Choose your preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for a Medicare Advantage health plan
- Review patients medical history and current conditions
- Assess medication history and provide relevant education
- Identify care gaps and recommend solutions
- Visits may include annual checkups or post-discharge follow-ups
- No treatment plan creation or medication prescribing required
Required Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Six months or more of recent experience as a Nurse Practitioner is preferred
- Master of Science in Nursing (MSN) from an accredited institution
In addition to professional growth and flexible scheduling, youll earn between $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
