This per diem opportunity allows you to set your own schedule, with shifts available any day of the week and a minimum weekly or bi-weekly commitmentperfect for balancing work with other priorities.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members throughout Warren County, including McMinnville, Morrison, and Centertown.
- Assess patients prior and current medical histories, ongoing medical conditions, and review medication usage.
- Provide patient education regarding medical conditions and proper medication administration.
- Identify care gaps and recommend actions to enhance overall patient outcomes.
- Focus on assessment and education; creating treatment plans or prescribing medication are not required.
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- At least 6 months NP experience preferred
- MSN from an accredited nursing school
