Make a direct impact in patient care by performing home health risk assessments in Warren County, TN. Work on your terms with a per diem, flexible schedule as a Nurse Practitioner dedicated to serving Medicare Advantage plan members in communities such as McMinnville, Morrison, and Centertown.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Choose your desired minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments, with each visit averaging about 45 minutes
- Evaluate patients prior and current medical histories, including medical conditions and medications
- Educate patients on their health conditions and medications
- Identify gaps in care and offer recommendations to enhance patient health
- Assessment types include annual evaluations and post-discharge visits
- There is no requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing school
- Current CPR/BLS certification
- At least 6 months of recent Nurse Practitioner experience (preferred)
