Take on a rewarding opportunity as a Nurse Practitioner delivering high-quality home health risk assessments for Medicare Advantage plan members across Monongalia County. This per diem position offers flexibility in your schedule, allowing you to choose shifts throughout the week and tailor your workload to your availability.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: select your own hours
- Minimal weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patients homes
- Evaluate prior medical history, active conditions, and medication regimens
- Educate patients on ongoing medical conditions, medication adherence, and healthy lifestyle strategies
- Identify care gaps and recommend solutions to improve patient health outcomes
- Visits may include annual assessments or post-discharge check-ins
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in West Virginia
- Board certification through the AANP or ANCC
- Graduate of an accredited Masters of Science in Nursing (MSN) program
- Current BLS (CPR) certification
- At least six months of Nurse Practitioner experience preferred
This position includes compensation of $80-$90 per evaluation, along with a mileage stipend and additional administrative time for your expertise and dedication.
