Job Description

Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments throughout Burke County, NC. This role involves visiting Medicare Advantage plan members in their homes, providing essential evaluations to support quality care transitions and preventative health.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling tailor your hours to fit your lifestyle
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments with each session lasting approximately 45 minutes
  • Evaluate patients prior and current medical history, including medical conditions and medication use
  • Deliver clear education to patients regarding their health conditions and prescribed medications
  • Identify gaps in patient care and make appropriate recommendations to encourage optimum health outcomes
  • No prescribing of medications or creation of treatment plans required

You will be traveling to various areas within Burke County, such as Morganton, Hickory, and Valdese, covering a diverse and appreciative patient population during each shift.

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of six months of NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
