Utilize your advanced nursing expertise and make a difference in the community through in-home risk assessments. In this per diem opportunity, Nurse Practitioners will conduct home health evaluations for Medicare Advantage plan members across Scranton, Carbondale, Dunmore, and other areas within Lackawanna County. Each visitwhether an annual health assessment or a post-discharge evaluationtypically lasts about 45 minutes, bringing valuable care directly to patients in their residences.

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members.
• Evaluate current and prior medical history, medical conditions, and medication use.
• Provide clear education on diagnoses and medications to members and caregivers.
• Identify and document any care gaps, recommend solutions, and escalate as necessary.
• No requirement to generate treatment plans or prescribe medications.

Qualifications:

• Active Nurse Practitioner license in Pennsylvania.
• Board Certification from AANP or ANCC.
• Current CPR certification (BLS).
• MSN from an accredited nursing program.
• At least 6 months of recent experience as a Nurse Practitioner is highly preferred.
• Competence and comfort working autonomously in home health environments.
