Job Description

Healthcare professionals with home health experience are invited to take on a per diem opportunity as a Nurse Practitioner performing risk assessments. In this role, youll conduct home visits to Medicare Advantage plan members in Concord, Kannapolis, Locust, and surrounding areas within Cabarrus County. Flexible scheduling is available, with the ability to create shifts to fit your availability between Monday and Sunday. A minimum hourly commitment per week or bi-weekly period is required.

Key Responsibilities:

  • Perform annual risk assessments during home visits for members enrolled in Medicare Advantage
  • Review and evaluate prior and current medical histories, existing conditions, and medication regimens
  • Educate patients on proper medication use and management of medical conditions
  • Identify gaps in patient care and propose recommendations to address those gaps
  • No requirement to create treatment plans or prescribe medication

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program

This role offers a pay rate of $80-$90 per evaluation, with additional compensation for mileage and administrative time.
