Job Description

Take on an impactful, patient-centered role as a Nurse Practitioner specializing in home health assessments on a per diem basis. Youll be instrumental in evaluating the health and wellness of Medicare Advantage plan members in various communities across Hendry Countysuch as Clewiston, LaBelle, and Monturaby conducting thorough, in-home risk assessments.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Self-scheduled: design a work calendar that fits your life
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care, providing actionable recommendations for improvement
  • Participate in post-discharge or annual health visits as needed
  • Visits average 45 minutes with no responsibility for prescribing medications or creating new treatment plans

Qualifications:

  • Active Nurse Practitioner license in the state of Florida
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience (preferred)

Compensation for this role ranges from $80$90 per evaluation, with additional mileage stipend and paid administrative time.
