Job Description

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingdesign your own workweek
  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Evaluate prior and current medical history, chronic conditions, and medication usage
  • Provide clear education on medical conditions and medications to promote patient understanding
  • Identify care gaps and offer actionable recommendations for follow-up
  • Engagements may include annual health assessments or post-discharge visits
  • No development of treatment plans or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current BLS/CPR certification
  • Board Certification from AANP or ANCC
  • Graduation with MSN degree from an accredited nursing program
  • At least six months of experience as a Nurse Practitioner preferred

This position compensates $80$90 per completed evaluation. Additional compensation is provided through a mileage stipend and payment for administrative time, recognizing your expertise and commitment to patient-centered care.
