Job Description
Take on a meaningful role providing high-quality care as a Nurse Practitioner in a home health setting, conducting risk assessments for Medicare Advantage plan members throughout El Paso County. This per diem opportunity is ideal for clinicians seeking flexibility, autonomy, and a focus on patient-centered assessments without the burden of ongoing treatment planning or prescribing.

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical histories, conditions, and medication profiles
- Deliver education to patients regarding medical conditions and prescribed therapies
- Identify and report care gaps; provide actionable recommendations
- Visits are typically 45 minutes and may include annual assessments or post-discharge evaluations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Licensed Nurse Practitioner in Colorado
- Current CPR certification (BLS)
- Board certification with AANP or ANCC
- Minimum of six months NP experience preferred
- MSN degree from an accredited institution
This position provides compensation of $80$90 per evaluation, with additional stipends for mileage and administrative time. Providers may be scheduled in various areas within El Paso County, including Colorado Springs, Fountain, and Monument, based on coverage needs for each shift.
