Elevate your clinical skills by providing essential home health risk assessments for Medicare Advantage members. As a Nurse Practitioner specializing in home health, you'll play a vital role in supporting improved patient outcomesdelivering comprehensive assessments during annual evaluations or post-discharge visits. You'll collaborate with individuals in various Montrose County communities, with the flexibility to create your own schedule and tailor your commitments to your lifestyle.
Schedule:
- Flexible shifts available between Monday and Sunday
- Option to build your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments in client residences
- Gather and evaluate patients prior/current medical and medication history
- Educate members about their health conditions and prescribed treatments
- Identify gaps in preventive care and recommend follow-up actions
- Focus on evaluation and educationno prescribing or treatment plans required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR/BLS certification
- Board certification via AANP or ANCC
- MSN from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience is preferred
As part of this per diem opportunity, you'll receive compensation between $80$90 per completed evaluation, along with a mileage stipend and additional pay for administrative time.
