Job Description
Deliver essential healthcare to individuals in their homes as a Nurse Practitioner specializing in home health services in Montrose County, CO. This per diem opportunity allows you to perform meaningful work, conducting risk assessments and supporting the health and wellness of Medicare Advantage plan members throughout communities including Montrose and Naturita.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual risk assessments for Medicare Advantage plan participants in their homes
- Review and evaluate medical history, current health conditions, and medications
- Educate members on health conditions and proper medication usage
- Assess for and identify potential gaps in care, recommending actions for better outcomes
- Visits may include annual assessments or post-discharge follow-ups
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certified through AANP or ANCC
- Completion of an MSN from an accredited nursing school
- At least 6 months experience as a practicing Nurse Practitioner preferred
