This per diem position allows Nurse Practitioners to create their own schedules, making it ideal for those seeking autonomy and variety in their workweek. Shifts can be adjusted to fit your needs, with the option to choose from Monday through Sunday.
Schedule:
- Flexible shifts available any day, MondaySunday
- Self-scheduling to match your availability
- Minimum weekly or bi-weekly hourly commitment
As a clinician in this rewarding role, you will visit Medicare Advantage plan members in their homes throughout Montrose County, including areas such as Montrose and Naturita. During each approximately 45-minute visit, youll perform comprehensive annual or post-discharge health assessments.
Key Responsibilities:
- Conduct thorough home health risk evaluations for a Medicare Advantage plan
- Review and assess past/current medical history, conditions, and medication use
- Educate members on their health conditions and prescribed treatments
- Identify and address gaps in member care, providing expert guidance
- No requirement to formulate treatment plans or prescribe medications
Qualifications:
- Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certification (ANCC or AANP)
- MSN from an accredited nursing school
- At least 6 months of recent NP experience (preferred)
Youll earn $80-$90 for each completed assessment, with additional mileage reimbursement and compensation for administrative tasks associated with care delivery.
