Experience a dynamic per diem opportunity providing advanced patient care in a home health setting. This position focuses on conducting comprehensive health risk assessments for Medicare Advantage members across White County, including Monticello, Brookston, and Monon. Each assessment takes approximately 45 minutes and involves evaluating annual or post-discharge health needs.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage health plan members
• Review prior and current medical histories, health conditions, and medication use
• Educate patients on managing their medical conditions and medications
• Identify gaps in care and make recommendations for follow-up or additional support
• No direct treatment planning or prescription authority required

Qualifications:

• Active Nurse Practitioner License in Indiana
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of current experience as a Nurse Practitioner is preferred

Compensation: Earn $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
