This opportunity focuses on evaluating and supporting Medicare Advantage plan members throughout Monticello and the broader White County, Indiana area, including Brookston and Monon. Youll enjoy a flexible schedule, giving you control over your shifts and time commitment, with opportunities to work minimum hours weekly or bi-weekly.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to best fit your needs

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, typically lasting 45 minutes, for Medicare Advantage members
  • Review prior/current medical history, ongoing health conditions, and medication regimens
  • Educate patients on their health status and prescribed treatments
  • Identify and address care gaps, offering actionable recommendations
  • Visits may include annual health assessments or post-discharge check-ins
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active, unrestricted Nurse Practitioner license in Indiana
  • Current CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Minimum of 6 months recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
