Step into a rewarding Per Diem opportunity providing essential care and support to patients in their homes. As an Advanced Practice Provider specializing in home health, you will play a key role in promoting wellness for Medicare Advantage members across Drew County, including communities such as Monticello, Wilmar, and Winchester.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members in patients homes
• Assess patients medical histories, current health status, and medication regimens
• Educate individuals regarding chronic conditions, medication management, and overall health maintenance
• Evaluate potential gaps in care and suggest recommendations to optimize patient well-being
• Visits may be focused on yearly assessments or post-discharge follow-up
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Current CPR certification (BLS)
• Board certified by AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• Minimum 6 months of recent experience as an NP preferred
