Job Description

Deliver advanced care in a flexible, patient-centered environment as a Nurse Practitioner focused on home health risk assessments within Drew County. This per diem opportunity involves visiting Medicare Advantage plan members in their residences, with locations potentially including Monticello, Wilmar, and Winchester. Tailor your schedule according to personal and professional needs, with shifts available any day of the week and no minimum number of hours required per week or bi-weekly.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • No required minimum weekly or bi-weekly hours; commitment based on your availability

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Evaluate past and current medical history, medical conditions, and medication use
  • Educate patients on diagnosed medical conditions and their medications
  • Identify potential care gaps and provide actionable recommendations for patient follow-up
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Six months or more of experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited program
