Take your Nurse Practitioner expertise into the community with a flexible, per diem opportunity focusing on comprehensive home health risk assessments. This position supports Medicare Advantage members in San Miguel County, serving areas such as Las Vegas, Pecos, and Montezuma.
Schedule/Hours:
- Flexible scheduling with shifts available any day between Monday and Sunday
- Choose your own hours
- Commitment to a minimum number of hours per week or every other week
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan participants in their homes
- Assess prior and current medical history, medical conditions, and medication usage
- Deliver education and guidance to patients regarding their conditions and medications
- Identify gaps in care and provide clear recommendations for follow-up
- No prescription writing or creation of treatment plans is needed
Qualifications:
- Active Nurse Practitioner license in New Mexico
- Board certification as an NP through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing school
- Preferably, at least six months of current Nurse Practitioner experience
Enjoy pay rates between $80-$90 for each completed evaluation, and receive additional compensation for mileage and administrative time.
