Make a meaningful difference by providing home health risk assessments for Medicare Advantage plan members throughout Cumberland County, TN. In this per diem opportunity, Nurse Practitioners will carry out focused, one-on-one health evaluations, ensuring comprehensive support to patients residing in Crossville, Pleasant Hill, Crab Orchard, and the surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual health risk assessments in patients homes
- Review prior and current medical history, documented conditions, and medication lists
- Deliver educational guidance regarding medical conditions and prescribed medications
- Identify and address potential gaps in care, offering practical recommendations
- No requirement to create ongoing treatment plans or prescribe medications
Preferred Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board certification (AANP or ANCC)
- Valid CPR (BLS) certification
- Minimum of 6 months experience as a practicing Nurse Practitioner is preferred
- MSN degree from an accredited nursing program
This role offers $80-$90 per completed evaluation, in addition to mileage stipend and compensation for administrative time. The average home health risk assessment takes approximately 45 minutes to complete, providing you with both a professional challenge and flexibility to manage your own schedule.
