Bring your compassionate care and clinical expertise to a dynamic, community-based nursing role, conducting home health risk assessments for Medicare Advantage plan members across White County, TN. Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own assignment days
- Minimum weekly or bi-weekly hourly commitment to suit your needs
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments (approximately 45 minutes each) on Medicare Advantage plan members
- Review and evaluate patients comprehensive medical histories, current medical status, and medication use
- Deliver educational support to patients regarding medical conditions and medication regimes
- Identify and communicate any gaps in care and recommend follow-up actions
- Focus exclusively on assessments; no prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least six months of Nurse Practitioner experience preferred
Youll earn $80$90 per evaluation, with additional compensation for mileage and administrative time.
