Enjoy the flexibility of per diem work as an experienced Nurse Practitioner specialized in home health assessments. In this opportunity, you will deliver essential care services to Medicare Advantage plan members across Putnam County, TN, including Cookeville, Algood, and Monterey. This position allows you to tailor your own schedule, choosing shifts that work best for your lifestyle while making a meaningful impact on members health and wellness.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own working hours
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Evaluate patients prior and current medical histories, conditions, and medication usage
• Educate patients about their health status, ongoing conditions, and prescribed medications
• Identify gaps in care, providing clear recommendations to address these needs
• Visits may include annual wellness assessments or post-discharge check-ins
• No responsibility for prescribing medications or creating treatment plans

Desired Qualifications:

• Active Nurse Practitioner License in Tennessee
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least six months of recent NP experience preferred

This role is compensated at $80$90 for each evaluation completed, with an additional mileage stipend and payment for administrative time.
