Nurse Practitioners seeking per diem flexibility will find this home health role ideal for balancing professional growth with scheduling autonomy. In this position, you will conduct home health risk assessments on Medicare Advantage plan members across Overton County, ensuring a meaningful impact in communities such as Livingston, Hilham, and Alpine. Assessments typically last about 45 minutes and may range from annual health check-ups to post-discharge visits, allowing for variety and patient-centered care.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment based on your availability
Key Responsibilities:
- Complete comprehensive home health risk assessments for a Medicare Advantage health plan
- Review and evaluate patients medical histories, current conditions, and medication histories
- Educate patients on their medical diagnoses and medication regimens
- Identify gaps in care and recommend next steps to address healthcare needs
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of six months experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
