Deliver advanced patient care and make a meaningful impact through personalized home health risk assessments. This per diem position is ideal for an experienced Nurse Practitioner looking to expand their clinical skills while enjoying exceptional flexibility and autonomy.
Schedule:
- Enjoy flexible shifts available any day, Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment ensures work-life balance
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members throughout Cumberland County
- Assess each members prior and current medical history, conditions, and prescription usage
- Educate patients and families about diagnoses, ongoing conditions, and medication adherence
- Review patient records to identify gaps in care and make actionable recommendations
- Visits may include annual health reviews or post-discharge assessments
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
This role offers a competitive per evaluation rate between $80-$90, complemented by a mileage stipend and compensation for administrative time.
