As a Nurse Practitioner specializing in home health, you will be responsible for conducting comprehensive risk assessments in communities like Alamosa, Blanca, and Hooper. This per diem opportunity allows for exceptional flexibility, giving you control over your work-life balance while serving Medicare Advantage members.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule according to your availability
- Minimum weekly or bi-weekly hourly commitment
What youll do:
- Perform annual health risk assessments in patients homes for Medicare Advantage plan members
- Evaluate medical histories, current and prior conditions, and medication usage
- Educate patients on managing their conditions and proper use of prescribed medications
- Identify gaps in care and recommend solutions to enhance patient outcomes
- Complete timely, thorough assessment documentation
- No need to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in the state of Colorado
- Board Certification by AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
This role offers compensation of $80-$90 per patient evaluation, plus a mileage stipend and payment for administrative timereflecting your expertise and commitment.
