Job Description

Deliver expert care and make a meaningful impact by performing home health risk assessments in Alamosa County. This per diem, field-based opportunity is ideal for Nurse Practitioners dedicated to enhancing patient outcomes through comprehensive evaluations in the comfort of each members home.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to suit your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Complete a thorough review of each patients medical and medication history
  • Educate patients on their health conditions and medications to promote understanding and self-management
  • Identify gaps in care and recommend solutions to enhance patient well-being
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Graduate of an accredited MSN program
  • At least six months of current NP experience preferred

Enjoy a competitive compensation structure of $80-$90 per evaluation, along with a mileage stipend and administrative time for completed assessments. Youll serve patients in Alamosa, Blanca, Hooper, and neighboring areas within Alamosa County, with each appointment typically lasting 45 minutes.
