Deliver high-quality care and make a meaningful impact as a Nurse Practitioner performing home health risk assessments. This per diem opportunity is ideal for professionals seeking flexibility, autonomy, and the satisfaction of providing comprehensive assessments to Medicare Advantage plan members in Kanawha County, including areas such as Bluefield, Princeton, and Athens. Each home visit typically lasts about 45 minutes, with various types of assessments including annual health evaluations and post-discharge visits.

Key Responsibilities
- Conduct detailed annual home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, medical conditions, and medication history
- Educate patients on identified medical conditions and medications
- Identify gaps in care and recommend solutions to improve health outcomes
- No treatment plan development or medication prescribing necessary
Preferred Qualifications
- Active Nurse Practitioner license in West Virginia
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of six months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
