This per diem position serves communities within Mercer County, including Trenton, Princeton, and Hamilton. Professionals in this role will deliver high-quality healthcare assessments in patient homes, helping to bridge gaps in care and promote better health outcomes.

Schedule:

• Choose flexible shifts between Monday and Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
• Review medical histories and current conditions, including medication history and management
• Provide patient education on health conditions and prescribed medications
• Identify and document care gaps, offering appropriate recommendations to address them
• No responsibilities for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of current NP experience is preferred
