Shape patient care in a flexible, impactful way through home health risk assessments for Medicare Advantage members in Fentress County, TN. This per diem opportunity is ideal for a Nurse Practitioner seeking autonomy and variety in their schedule, serving residents across communities such as Jamestown, Allardt, and Grimsley.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own working schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct comprehensive annual risk assessments in patients homes (average of 45 minutes per visit)
- Assess patients prior and current medical history, ongoing conditions, and medication use
- Educate patients and caregivers about medical conditions and prescribed medications
- Identify any gaps in care and suggest actionable recommendations
- Perform visits for both annual health assessments and post-discharge evaluations
- No prescribing or treatment plan creation is involved
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS/CPR certification
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
