Job Description

Experienced Nurse Practitioners seeking flexibility and meaningful patient engagement will find this per diem opportunity both rewarding and accommodating. This role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members across White County, with visits centered in communities such as Monticello, Brookston, and Monon. You will play a vital part in supporting patient health journeys without the need for ongoing treatment or medication prescribing.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Create your own work schedule for ultimate autonomy
  • Choose a minimum weekly or bi-weekly commitment that matches your availability

Key Responsibilities:

  • Perform detailed annual home health risk assessments for a Medicare Advantage plan
  • Review and document patients medical histories, current health status, and medications
  • Educate patients about their health conditions and prescribed medications
  • Identify any gaps in care, offering suggestions to help patients and their primary care providers address them
  • Visits typically last about 45 minutes and may include annual assessments or post-discharge check-ins
  • No responsibility for developing treatment plans or prescribing medication

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months of current NP experience is preferred

This position offers compensation of $80$90 for each completed evaluation, along with stipends for mileage and administrative tasks. Youll enjoy significant scheduling freedom while making a positive impact on the well-being of Medicare members in your region.
