This opportunity offers the flexibility to tailor your schedule, with shifts available throughout the weekMonday through Sundayacross Linn County, including areas such as Albany, Lebanon, and Sweet Home.

Schedule:

• Flexible shifts spanning MondaySunday

• Commit to a minimum weekly or bi-weekly hour requirement to suit your availability

Core Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage members
• Review and evaluate patients current and past medical histories, including conditions and medication regimens
• Educate patients on their individual health conditions and medications, promoting wellness and improved self-care
• Identify possible gaps in care, offering recommendations for additional services or follow-up as needed
• Note: This role does not involve direct prescribing or treatment plan creation

Qualifications:

• Active Nurse Practitioner License (OR)
• MSN from an accredited nursing institution
• Current CPR certification (BLS required)
• Board Certification via AANP or ANCC
• Minimum of 6 months experience working as a Nurse Practitioner preferred
