This per diem role provides autonomy to create your own schedule with shifts available any day of the week, offering the chance to balance work and life on your terms.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose weekly or bi-weekly minimum commitments
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members in the Monett area and throughout Barry County
- Gather and evaluate a patients medical and medication history, along with prior and current medical conditions
- Deliver patient-specific education on chronic conditions and prescribed medications
- Identify any gaps in care, providing clear recommendations and resources for resolution
- Assessments may be annual reviews or post-discharge visits
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Nurse Practitioner license valid in Missouri
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- MSN from an accredited nursing school
- At least six months of recent Nurse Practitioner experience preferred
