Job Description

Play a pivotal role in supporting the well-being of your community by conducting home health risk assessments for Medicare Advantage plan members throughout Sussex County. This rewarding per diem position offers exceptional flexibility, control over your schedule, and compensation for both clinical and administrative efforts.

Schedule & Work Arrangement:

  • Flexible shifts available any day, Monday through Sunday
  • Set your own workdays and hours
  • Meet minimum weekly or bi-weekly hourly requirements to suit your lifestyle

Role Details:

  • Perform comprehensive home health risk assessments, averaging 45 minutes per visit
  • Provide in-home care around Sussex County, including Seaford, Milford, and Georgetown
  • Visits may include annual health assessments or post-discharge evaluations
  • Review medical history, current conditions, and medications with members
  • Educate patients on their health conditions and medications, enhancing their understanding and involvement in care
  • Identify gaps in care and provide recommendations for resolution, improving member outcomes
  • No treatment plan development or medication prescribing required

Desired Qualifications:

  • Active Nurse Practitioner license in Delaware
  • MSN from an accredited nursing program
  • Board certification (AANP or ANCC)
  • Current BLS/CPR certification
  • Six months or more of nurse practitioner experience preferred

Compensation & Perks: For each evaluation completed, receive between $80$90, with additional compensation for mileage and administrative duties, recognizing your time spent both in travel and documentation.
