Step into an important, community-based nurse practitioner role delivering home health assessments throughout Sussex County. This per diem opportunity offers exceptional flexibility, allowing you to create a schedule that best fits your lifestyle, with shifts available any day of the week. Youll perform focused home health risk assessments for Medicare Advantage plan members living in areas including Milford, Seaford, and Georgetown.
Schedule:
- Flexible shifts offered Monday through Sunday
- Set your own weekly or bi-weekly commitments
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments at patients residences
- Review and evaluate prior/current medical history, medical conditions, and prescribed medications
- Educate patients on their health conditions and medication regimens
- Identify care gaps and recommend next steps to close those gaps
- No prescribing or treatment regimensfocus is on assessment and education
Qualifications:
- Active and unrestricted Nurse Practitioner license in Delaware
- Board Certified (AANP or ANCC)
- Minimum MSN from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months recent experience as an NP preferred
- Experience or interest in home health care highly valued
