Job Description

Deliver high-quality care and peace of mind to patients by conducting home health risk assessments as a Nurse Practitioner in Transylvania County, NC. This per diem position offers the opportunity to work with Medicare Advantage plan members throughout the area, including Brevard and Rosman, ensuring comprehensive clinical evaluations and personalized patient educationall within the comfortable setting of your patients homes.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Choose your own schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments tailored to individual patient needs
  • Evaluate and document medical histories, current conditions, and medication use
  • Educate members thoughtfully about their medical conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations to improve health outcomes
  • No treatment plan creation or prescribing involved

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Current CPR certification (BLS required)
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred
