This per diem role focuses on performing home health risk assessments for Medicare Advantage plan members in Transylvania County, including Brevard and Rosman. As a Nurse Practitioner in this position, you will deliver short, focused health assessments with no treatment planning or prescribing responsibilities, making this an excellent opportunity for those seeking work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule based on availability
- Choose your minimum weekly or bi-weekly hourly commitment
Core Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage health plan members in their homes
- Review and evaluate patients prior and current medical history, medical conditions, and medication history
- Educate patients on their medical conditions and medication regimens
- Identify gaps in care and make recommendations to promote health and wellness
- Conduct evaluations in various locations throughout Transylvania County, as scheduled per shift
Preferred Qualifications:
- Active Nurse Practitioner license in North Carolina
- MSN from an accredited nursing school required
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months current experience as a Nurse Practitioner preferred
- Previous home health experience a plus
