This per diem role focuses on performing home health risk assessments for Medicare Advantage plan members in Transylvania County, including Brevard and Rosman. As a Nurse Practitioner in this position, you will deliver short, focused health assessments with no treatment planning or prescribing responsibilities, making this an excellent opportunity for those seeking work-life balance.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule based on availability
  • Choose your minimum weekly or bi-weekly hourly commitment

Core Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage health plan members in their homes
  • Review and evaluate patients prior and current medical history, medical conditions, and medication history
  • Educate patients on their medical conditions and medication regimens
  • Identify gaps in care and make recommendations to promote health and wellness
  • Conduct evaluations in various locations throughout Transylvania County, as scheduled per shift

Preferred Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • MSN from an accredited nursing school required
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Minimum of 6 months current experience as a Nurse Practitioner preferred
  • Previous home health experience a plus
