Advance your clinical skills by contributing to the well-being of Medicare Advantage members through comprehensive home health risk assessments in Shelby County, TN. This position offers meaningful patient interaction while supporting preventive health measures.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own work schedule
- Minimum weekly or bi-weekly hours to be agreed upon
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication history
- Educate patients about their medical conditions and prescribed medications to support understanding and adherence
- Identify any gaps in care and offer recommendations to address these issues
- Conduct assessments that may vary from annual wellness checks to post-discharge follow-up visits
- No prescribing or creation of treatment plans required
Desired Qualifications:
- Active Nurse Practitioner License in TN
- Current Basic Life Support (BLS)/CPR certification
- Board Certification through AANP or ANCC
- Minimum of 6 months clinical experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
