Make a meaningful impact on patient care by providing comprehensive home health risk assessments to Medicare Advantage members across Perry County. This per diem opportunity is ideal for a Nurse Practitioner who values flexibility, autonomy, and the chance to deliver patient-centered care in a home setting.

Schedule:

Responsibilities:

• Complete in-home annual risk assessments for Medicare Advantage plan members
• Review prior and current health history, medical conditions, and medications
• Offer patient education regarding chronic conditions and treatment regimens
• Identify care gaps and make informed recommendations for follow-up actions
• No responsibility for creating treatment plans or prescribing medications

What Youll Need:

• Active Nurse Practitioner license in Missouri
• MSN degree from an accredited nursing program
• Board certification (AANP or ANCC)
• Current CPR certification (BLS)
• At least 6 months of current Nurse Practitioner experience preferred
