As a Nurse Practitioner specializing in home health risk assessments, your expertise will directly support patient wellness and care continuity throughout Wilkes County. With flexible scheduling and the ability to create your own shifts between Monday and Sunday, this role is ideal for practitioners seeking both autonomy and a meaningful patient-centered approach.

Schedule:

• Flexible shifts available any day of the week
• Create your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive annual health assessments and post-discharge visits for Medicare Advantage plan members within Wilkes County.
• Evaluate patients current and prior medical histories, conditions, and medication usage in their homes.
• Provide personalized education on health conditions and medication management.
• Identify care gaps and recommend actions to improve overall patient outcomes.
• No prescribing medications or creating treatment plans requiredfocus is strictly assessment and education.

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Minimum of 6 months recent NP experience preferred

This position offers compensation of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
