Job Description
Seeking an experienced Nurse Practitioner to provide essential home health risk assessments for Medicare Advantage plan members throughout Kent County. This per diem position is highly flexible, allowing you to create your own schedule with shifts available between Monday and Sunday. Shifts may be scheduled in Dover, Smyrna, or Milford, and youll complete assessments directly in patients homes.
Schedule:
- Flexible shifts available seven days a week
- Self-scheduling to accommodate your availability
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review patients medical history, current medical conditions, and medication history
- Educate patients on their medical conditions and medication regimens
- Identify and document gaps in care, recommending appropriate next steps for resolution
- Complete all documentation accurately; no ongoing treatment planning or prescription management required
Qualifications:
- Active Nurse Practitioner License in Delaware
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of current Nurse Practitioner experience preferred
- Excellent interpersonal, organizational, and assessment skills
This role offers compensation of $80$90 per completed evaluation, plus a mileage stipend and administrative time.
