Make a meaningful impact by providing essential healthcare services to Medicare Advantage plan members in Gibson County. As a Nurse Practitioner specializing in home health risk assessments, you will visit clients in various locations, including Humboldt, Milan, and Trenton. Each evaluation typically takes about 45 minutes and may consist of an annual wellness assessment or a post-discharge visit.
Schedule:
- Flexible shifts available from Monday through Sunday
- Choose your own working days and hours
- Minimum weekly or bi-weekly hourly commitment required
You will play a vital role in identifying health risks, supporting continuity of care, and educating patientsall within the comfort of their homes.
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients' past and current medical histories, conditions, and medication usage
- Educate patients on their conditions and prescribed medications to encourage health awareness and compliance
- Detect gaps in care and offer actionable recommendations to improve patient outcomes
- Focus exclusively on assessments; no requirement to create treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner License valid in Tennessee
- Active CPR (BLS) certification
- Board Certification via AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience preferred
This is a per diem opportunity offering $80$90 per evaluation, along with mileage stipend and compensation for administrative time.
