A dynamic opportunity is available for an experienced Nurse Practitioner interested in home health risk assessments within Gibson County, TN. This per diem role involves visiting Medicare Advantage members in their residences throughout communities such as Humboldt, Milan, and Trenton. Each evaluation averages 45 minutes and may include annual health assessments or post-discharge visitshelping ensure continuity of care and patient well-being.
Schedule:
- Flexible shifts available seven days a week

- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Perform annual health risk assessments for Medicare Advantage members in home settings
- Review medical history, current conditions, and medications
- Educate patients on chronic conditions and prescriptions
- Identify any gaps in care and recommend solutions
- No requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS (CPR) certification
- Board Certified through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of Nurse Practitioner experience preferred
