Job Description

An opportunity is available for a Nurse Practitioner with home health experience to conduct risk assessments for Medicare Advantage plan members across Cibola County, NM. This per diem position offers the flexibility to tailor your schedule, with shifts available throughout the week and the ability to create your own workdays. Assessments typically take approximately 45 minutes and may include annual health reviews or post-discharge evaluations in patients homes located in Grants, Milan, Acoma Pueblo, and surrounding communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete in-home annual health risk assessments for Medicare Advantage members
  • Assess patients current and previous medical histories, ongoing conditions, and comprehensive medication usage
  • Deliver patient education regarding conditions and prescribed medications
  • Identify care gaps and recommend actionable follow-up steps
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New Mexico
  • Current Basic Life Support (CPR) certification
  • Board certification through AANP or ANCC
  • At least 6 months of recent experience as a Nurse Practitioner (preferred)
  • Master of Science in Nursing (MSN) from an accredited institution
