In this position, you will conduct home health risk assessments for Medicare Advantage plan members throughout Leon County, Florida, including areas such as Midway, Tallahassee, and Woodville. You'll have the autonomy to create your own schedule, making this an ideal opportunity to balance professional growth and personal commitments.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to set your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review medical history, current conditions, and medication history during each home visit
• Educate patients about their medical conditions and medications
• Identify and document gaps in care, providing recommendations for improvement
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Florida
• Board certified through AANP or ANCC
• Current BLS (CPR) certification
• Master of Science in Nursing (MSN) from an accredited institution
• Minimum of six months of Nurse Practitioner experience preferred

You'll be compensated $80$90 per completed evaluation, and will also receive a mileage stipend along with payment for administrative time, ensuring your work is valued both in the clinic and on the road.
