Deliver compassionate, high-quality care as a Nurse Practitioner specializing in home health risk assessments within Gadsden County, FL. This per diem role is ideal for clinicians looking for flexible, independent work in a community-based setting, with opportunities to serve patients in areas such as Quincy, Chattahoochee, and Havana.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit personal preferences
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual health risk assessments for Medicare Advantage plan members in their homes
- Assess prior and current medical history, chronic conditions, and medication usage
- Educate patients on medical diagnoses and proper medication management
- Detect gaps in care and provide actionable recommendations for follow-up
- Focus exclusively on assessmentsno prescribing or treatment plan development required
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Board certification through AANP or ANCC
- Minimum 6 months of recent Nurse Practitioner experience preferred
- Graduate of an accredited MSN program
- Current BLS/CPR certification
