Elevate your clinical expertise by providing comprehensive in-home health assessments for Medicare Advantage plan members across Leon County, FLincluding Tallahassee, Woodville, and Midway. This per diem opportunity allows Nurse Practitioners to perform rewarding work with the flexibility to design their own schedules and shifts, offering an ideal balance between professional engagement and personal commitments.
Schedule:
- Flexible shifts available seven days a week (MondaySunday)
- Customize your work schedule to fit your lifestyle
- Commit to as many or as few hours as desired on a weekly or bi-weekly basis
Key Responsibilities:
- Perform in-depth home health risk assessments, typically lasting about 45 minutes per visit
- Review and evaluate members past and current medical histories, conditions, and medication regimens
- Deliver patient education regarding health concerns and medication management
- Identify care gaps and provide clear recommendations to support improved health outcomes
- No requirements for treatment planning or prescribing medication
Qualifications:
- Unrestricted Nurse Practitioner license in Florida
- Board Certification from AANP or ANCC
- MSN degree from an accredited nursing program
- Up-to-date CPR (BLS) certification
- Minimum of 6 months experience as a Nurse Practitioner preferred
- Strong communication skills and ability to work independently
