Job Description
Make a meaningful impact on patient well-being by performing in-home risk assessments for Medicare Advantage plan members across Henry County, IN. This per diem Nurse Practitioner position offers exceptional scheduling flexibilitycreate your own schedule with shifts available any day of the week, including weekends, and meet only a minimum weekly or bi-weekly commitment.
Schedule:
- Flexible shifts between Monday and Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive in-home annual health risk assessmentslasting approximately 45 minutes eachfor Medicare Advantage members
- Review detailed medical history, ongoing conditions, and medication use
- Educate individuals on their diagnoses and medication regimens
- Identify care gaps and provide actionable recommendations to enhance their health journey
- No treatment planning or medication prescribing required
Qualifications:
- Current Nurse Practitioner license in Indiana
- Valid CPR certification (BLS)
- Board certification as an NP through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing from an accredited institution
