Make a meaningful impact on patient care in a per diem, community-based nurse practitioner role focused on home health risk assessments. This opportunity offers flexibility and autonomy, allowing you to create your own schedule while conducting comprehensive in-home evaluations within Dane County, including Madison, Sun Prairie, and Fitchburg.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate medical history, current health conditions, and medications
• Educate patients on managing their medical conditions and understanding medications
• Identify care gaps and make recommendations to optimize patient health
• Visits may include annual assessments and post-discharge evaluations
• No responsibilities for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Wisconsin
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC required
• At least 6 months of current experience as a nurse practitioner preferred
• MSN degree from an accredited nursing school

This position compensates $80-$90 per evaluation, with additional mileage reimbursement and administrative time included.
