Utilize your expertise as a Nurse Practitioner to make a meaningful impact in the lives of Medicare Advantage plan members through in-home health risk assessments. In this per diem opportunity, you will visit individuals in Middleport and surrounding areas throughout Meigs County, such as Pomeroy, Syracuse, and Racine. Each home visit is designed to promote preventive care and support members' well-being.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their residences
• Review and evaluate patients medical histories, current medical conditions, and medication regimens
• Educate members on their medical conditions and appropriate medication usage
• Identify gaps in care and provide actionable recommendations for improved health outcomes
• No requirements for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in OH
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• Minimum of 6 months current NP experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
